what are the different levels?

Everything is different and taken on a case by case basis.

In general, we give 1-2 warnings before someone gets a week vacation, but there is no discrete rule. Time between warnings as well as severity and reason are all taken into account, among other things.

shouldn't this be public knowledge? for all we know warning level of "100" is ok. Nothing is mentioned at all anywhere. not even in the FAQs.
