The German film Cannibal. Made me so sick I didn't eat meat for a week.  Even fast-forwarded through parts. I don't recommend it. Not because it's bad, but because I don't know if anyone could take it. If I'd watched it while I had the flu I would have thrown up for sure  .

It is somewhat problematic since, if I know I am about to see a disgusting work of art, then it definitely raises my expectations, but I have seen "Cannibal Holocaust" and Pasolini's "Salo, or the 120 Days of Sodom". They might be in the right direction. The disgusting thing about "Holocaust" is not the overt violence, but the character of the encroaching protagonists, and of course the animal kills.
I've also been treated to clips of "Ichi the Killer".
On a purely cerebral sense with nothing explicit on view, I was totally disgusted with Fellini's "Satyricon" and "Amarcord". Satyricon in particular congers a certain punch drunk moroseness for me, probably due to the hopelessness.
And Bram Stoker's Dracula was totally disgusting by its overt faithlessness to the book, though that is probably not what you are looking for.

The Devil's Rejects, Last House on the Left (2009), The Texas Chainsaw Massacre (1974), The Hills Have Eyes (2006), and Eden Lake. I know I've seen others too, but I can't bring them to mind.

Don't Torture a Duckling, Passion of the Christ, Hannibal (One of the most unnecessarily gruesome films I ever saw before I was 15) There are a lot of scenes I find disgusting or shocking, but the rest of the film isn't. (That basically describes every Haneke film)

You guys may laugh at me, but... Hostel 2 is like the grossest movie ever. The part where the chick was tied up and she was being cut up and the girl was masturbating with her blood... Just... Ugh.

Well, in that regard you can call every documentary footage filmed at the KZs (for example the footage Sam Fuller shot in Falkenau) or even the found footage in Harun Farockis Essay on an Übergangslager called "Aufschub" just as shocking but you're right.

I like the film but it is definitely just heartbreaking, shocking, and disgusting in terms of ones feelings.
Also both versions of Funny Games
I just hated what happened in the story and the evil people in it. Nothing redeeming about it and people say Schindler's List is depressing. Think again.

I almost shut it off during the first part as I thought it was bad, BUT then the second half was really good. After the movie ended I watched the first part again. It made more sense then  .

I won't say it's the grossest movie ever, but that has to be one of the most disturbing scenes in a mainstream horror film I've ever seen. That part was so fucking sick. I liked Hostel Part II (It's miles better than the first), but haven't been in a hurry to watch it again due to that scene. It was just too hard to watch. Speaking of Hostel, I wonder if they'll make a 4th. I liked the third. I would like to see a final chapter where this organization is taken down.
